The first half of this decade was the heyday of the PFI - brand-new hospitals and health facilities were built all over the UK, paid for by the private sector and guaranteed by the NHS, in a national building programme valued at over £11bn. Since then, the economy has dipped and other models have arisen. Developers and medical professionals worked together to upgrade GP health centres under LIFT, while the Independent Sector Treatment Centres (ISTC) programme saw the private sector both building and running health facilities.

Localised decision-making is the current government ethos. The trend is towards cost saving, public-sector reorganisation, the abolition of Strategic Health Authorities and Primary care Trusts, and new commissioning and delivery patterns, with a new role for the private sector.
